What Is an IFSP (Individualized Family Service Plan)

If your child is eligible for services through Early Intervention, the next step is to develop an IFSP. Your IFSP must be completed within 45 days after your child’s referral to Early Intervention. The IFSP is written at the IFSP meeting which is held at a time and place convenient for you and your family. You, your service coordinator and a representative from the evaluation team will be present. At this time the evaluations will be reviewed and your concerns and goals for your child will be discussed. As a team, you will work on strategies, activities and services that will lead to the outcome you hope to reach.

As your child grows and changes, so will their goals. Six month reviews of the IFSP and annual meetings to reevaluate the IFSP are designed to update and provide new outcomes and drop old ones that have been reached. New strategies and activities to help meet new outcomes will be discussed.

You may request a review of your child's IFSP at any time. Your service coordinator can set up a meeting to discuss any new concerns you may encounter.
